Registrations received after September 15, 2008 will be charged a $50.00 late fee.

REFUND POLICY- IASN will refund conference fees less a $25.00 processing fee until September 15, 2008. After September 15, 2008 no refunds will be given.
The Monday evening dinner is for all conference attendees. Please indicate on the registration form if you will be attending the dinner.
The Diabetes Workshop is limited to 60 participants and is first come, first serve. Please indicate on the registration form if you will be attending that session.
